The registration form and Image Release authorization form for the upcoming All About Kids summer production is located below.
Download the forms to your computer and print. Return the completed form to the church office with your payment.
Please note that both the registration form and the image release form are required to be completed and submitted before the May 23rd deadline to recieve the $50 discount.
